Collaborative Care
Working Togeather
We value building relationships with local services, organisations and businesses who share our values and purpose to provide care and support.

Collaborative Care - Partnering with Agencies to provide client-centred support
We understand that only part of an individual's care happens in the therapy room. With a client's consent we can work collaboratively to support meaningful engagement with our service.
Collaborative care could look like anything from ensuring the appointment is at a time where transport is available, to attending a care team meeting, to facilitating an initial meet and greet or warm handover. Collaborative care is always delivered with an emphasis on maintaining confidentiality and making the client central to the process.

Who Do We Work With?
We have existing relationships with agencies that can in some circumstances grant client’s access to funded sessions.
These include:
- NDIS: For self-managed or plan-managed participants on an NDIS plan.
- VOCAT: For those awarded counselling through Victims of Crime Assistance Tribunal. We can also assist by providing VOCAT reports.
- Open Arms: We are registered with BUPA as an Open Arms provider.
- Melbourne City Mission: For young people aged 16-25 experiencing or at risk of homelessness, sessions may be accessible through Melbourne City Mission FRMP brokerage. Contact us to learn more.
